PROTECCIÓN DE DATOS PERSONALES

Que, de conformidad con lo establecido en la Ley N° 29733 y su Reglamento, declaro haber sido informado y autorizo que los datos personales y/o sensibles consignados en el presente formulario, así como los que se obtengan en virtud de mi relación con la Universidad de Piura, (en adelante LA UNIVERSIDAD), incluida mi imagen, voz, firma, firma electrónica y/o huellas dactilares, sean incorporados y almacenados en el Banco de Datos de LA UNIVERSIDAD, u otros que cumplan con las medidas de seguridad requeridas por la normativa de la materia; y sean tratados por LA UNIVERSIDAD y/o por personas naturales y/o jurídicas a quienes la misma delegue o encargue.  En este sentido, LA UNIVERSIDAD y/o las personas naturales y/o jurídicas a quienes la misma delegue o encargue podrán tratar dichos datos con las siguientes finalidades: (i) para la ejecución de la relación con el titular de los datos, (ii) para realizar la gestión económica, contable y de cobranza de los pagos efectuados por los participantes, (iii) para establecer y/o mantener contacto e informar y/o promocionar las actividades propias de LA UNIVERSIDAD, (iv) para fines estadísticos y/o de investigación, (v) para fines de prospección comercial, de marketing y/o envío de publicidad e información sobre productos y servicios, (vi) para ser compartidos con la comunidad UDEP-Alumni y cualquier otra unidad académica o administrativa de LA UNIVERSIDAD. Los datos serán tratados mientras se consideren necesarios para los fines señalados. LA UNIVERSIDAD deja constancia de la posibilidad de ejercer los derechos de acceso, rectificación, cancelación y oposición, y cualesquier otros previstos, conforme a la normativa de la materia, solicitándolo por escrito en las siguientes direcciones: Av. Ramón Mugica 131, Urb. San Eduardo, Piura o Calle Mártir José Olaya 162, Miraflores, Lima. En consecuencia, por la presente autorizo a la Universidad de Piura a realizar las actividades antes señaladas.

That, in accordance with the provisions of Law No. 29733 and its Regulations, I declare that I have been informed and authorize that the personal and/or sensitive data contained in this form, as well as those obtained by virtue of my relationship with the University of Piura, (hereinafter THE UNIVERSITY), including my image, voice, signature, electronic signature and/or fingerprints, are incorporated and stored in the Database of THE UNIVERSITY, or others that comply with the required security measures by the regulations of the matter; and are treated by THE UNIVERSITY and/or by natural and/or legal persons to whom it delegates or commissions. In this sense, THE UNIVERSITY and/or the natural and/or legal persons to whom it delegates or commissions may process said data with the following purposes: (i) for the execution of the relationship with the owner of the data, (ii) ) to carry out the economic, accounting and collection management of the payments made by the participants, (iii) to establish and/or maintain contact and inform and/or promote the activities of THE UNIVERSITY, (iv) for statistical purposes and/ or research, (v) for purposes of commercial prospecting, marketing and/or sending advertising and information on products and services, (vi) to be shared with the UDEP-Alumni community and any other academic or administrative unit of THE UNIVERSITY . The data will be processed as long as they are considered necessary for the purposes indicated. THE UNIVERSITY records the possibility of exercising the rights of access, rectification, cancellation and opposition, and any other provided, in accordance with the regulations on the matter, requesting it in writing at the following addresses: Av. Ramon Mugica 131, Urb. San Eduardo, Piura or Calle Martir José Olaya 162, Miraflores, Lima. Consequently, I hereby authorize the University of Piura to carry out the aforementioned activities.
