In accordance with Law No. 29733, Personal Data Protection Law and its Regulations, I declare that I have been informed by the University of Piura (LA UNIVERSIDAD) with RUC No. 20172627421 and legal address at Av. Ramon Múgica N° 131 Urb. San Eduardo (El Chipe Sector) of the department, province and district of Piura, and I authorize that the personal data related to me, as well as the person whom I represent or the person I have referred as a contact, recorded in this form, as well as those obtained by virtue of my relationship with THE UNIVERSITY, including my image, voice, signature, electronic signature, fingerprints, medical and/or psycho-pedagogical results, are stored in the Data Banks of THE UNIVERSITY[1] or others that comply with the security measures required by the regulations on the matter; and are processed by THE UNIVERSITY and/or by natural and/or legal persons whom it delegates or entrusts; who may process said data for the following purposes:

  2. To provide the university higher education service at the national or international level.
  3. For the execution of the relationship with the owner of the data, his representative and/or the person referred to as contact, for the purposes of:

    2. To inform the activities of THE UNIVERSITY.
  4. To manage registration in associative, cultural, recreational and sports events and activities, organized by THE UNIVERSITY.
